Pretransfusion Testing and Antibody Identification Program Overview

Program Description

Pretransfusion testing must be performed correctly to ensure patient safety during the transfusion process. This program reviews the AABB Standards related to pretransfusion testing. When the antibody detection test is positive, antibody identification must be performed to determine the antibody specificity. It is important to understand the immune response and to be able to recognize a primary or secondary response. This response has an impact on the immunoglobulin class of the antibody being produced and must be considered if the patient is experiencing post transfusion red cell destruction. The blood group system antigen and antibody characteristics are discussed and knowing the characteristics aids in antibody identification. Once the antibody has been identified, transfusion service staff must decide the clinical significance of the antibody to determine if known antigen negative Red Blood Cells must be supplied for transfusion.

  • Demonstrate the process of including and excluding antibody specificity(ies) when performing antibody identification
  • Discuss appropriate crossmatch technique
  • Assess antibody significance to make transfusion recommendations
